School enrollment requirements in Florida

Florida law requires certain immunizations are given prior to entry into schools and childcare centers. DOH-Lee can provide those required immunizations.
Transfering Immunization Records
If coming from another state and your child is enrolling in Florida schools, be sure to check the required immunizations. Immunization requirements can be different from state to state.
- Transfer requests can be faxed to 239-332-9517 or emailed to DOHLeeImmunizations@FLHealth.gov.
- Transfer requests will be completed within 5 business days.
Exemptions
Immunization religious exemptions (DH Form 681). Individuals requesting Religious Exemptions for children need to apply in person and bring the child’s birth certificate and parent/guardian ID.
