Having anyone tattooing in a tattoo shop without an active tattoo artist license, subjects the business to a fine and cancellation of their tattoo establishment license.
A guest tattoo artist registration is required for any tattoo artist who:
- Plans to tattoo in Florida.
- Has a valid artist license, registration, or certification issued by a jurisdiction outside of Florida.
A guest tattoo artist registration is valid for up to 14 consecutive days from the date of issue.
To apply for a guest tattoo artist registration in Lee County, you must submit the following items to DOH-Lee.
- Download and complete the Application for Guest Tattoo Artist Registration. Email the completed application to LeeEH@flhealth.gov.
- The guest tattoo artist registration must include the name of the licensed tattoo establishment(s) where the artist will work.
- Pay a $35 fee. You can pay online by visiting myfloridaehpermit.com or selecting “Pay Invoice.” Send a copy of a government-issued photo ID. Applicant must be at least 18 years old.
- Send proof of an active license, registration, or certification in a jurisdiction outside Florida.
- A guest artist may have already completed an education course and received a passing score of at least 70% on the test required for a license, registration, or certification in another jurisdiction.
- If so, they may submit the curriculum for the course and test to DOH-Lee. DOH-Lee will determine whether the course substantially meets or exceeds the requirements set by section 64E-28.006, F.A.C.
- If DOH-Lee determines that the curriculum does not substantially meet or exceed the requirements of section 64E-28.006, F.A.C., the guest artist will have to take the bloodborne pathogen and communicable diseases course . The guest artist must successfully pass with a minimum score of 70%. Please email the certificate of completion to LeeEH@flhealth.gov.
