Having anyone tattooing in a tattoo shop without a valid tattoo artist license, subjects the business to a fine and cancellation of their tattoo establishment license.

 

Applying for a Tattoo Artist License

To apply for a tattoo artist license in Lee County, you must submit the following items to DOH-Lee.  

  • Download and complete the Application for Tattoo Artist License . Email the completed application to LeeEH@flhealth.gov.
    • The address on the application must be the physical address of the tattoo artist in Lee County, regardless of where they work.
    • The application must include the name of the licensed tattoo business where the tattoo artist will perform or intends to perform tattooing.
  • Pay the $60 fee. You can pay online by visiting myfloridaehpermit.com or select “Pay Invoice.”
  • Send a copy of a government-issued photo ID. Applicant must be at least 18 years old.
  • Complete the required bloodborne pathogen and communicable diseases course for tattoo artists with a minimum of 70% passing score. Email the certificate of completion to LeeEH@flhealth.gov.
