Chapter 381.0065 F.S. requires a building that is in an area zoned or used for industrial or manufacturing purposes (or an equivalent use) that is served by an onsite sewage treatment and disposal system (OSTDS) may not be occupied until the owner or tenant has written approval from the Florida Department of Health (DOH).
A person who owns or operates a business or facility located in these areas must obtain an annual operating permit from DOH.
- Submit a completed application (Form DH 4081)
- Pay the $175.00 annual fee. Pay online by visiting MyFloridaEHPermit.com.
A business that proposes to occupy a building with a current operating permit must do the following to add the business to the existing operating permit.
- Submit a completed business survey (Form DH 4081A)
- Pay a $50.00 amendment fee. Pay online by visiting MyFloridaEHPermit.com.
