New OSTDS applications can now be submitted by email to LeeEH@flhealth.gov. We are only able to accept pdf files at this time. When submitting documents electronically the design professional must digitally sign and seal the documents and they must not be locked when the digital signature is applied. We must be able to apply our approval stamps. An invoice for payment will be returned to the email address that submitted the application, the invoice and can be paid online at  https://www.myfloridaehpermit.com/

Reviews will not begin until payment is received.

The Department of Health in Lee County has also prepared this packet of materials to help guide you through the permitting process for your new septic system. Following this information will ensure your application is appropriately reviewed. The application package must include:

  1. Application For Construction Permit:To apply for a septic permit, a complete application is required. The instructions accompanying this application explain what information is needed. Be sure all submitted forms have been signed and dated.  FEES: The OSTDS application fee in Lee County is $390.00. If it is determined that a site evaluation is needed, the applicant will be billed an additional $115.00. The Performance Based Treatment System application fee is $465.00. There are additional fees for inspections on the outer islands only accessible by boat. All fees are made payable to the Florida Department of Health in Lee County.
  2. SITE PLAN:The site plan requires all pertinent existing and proposed features included in the instructions found in the attachment.
  3. FLOOR PLAN: A floor plan drawn to scale or showing building area totals, all rooms labeled, and outside dimensions of each dwelling unit is required.  If the residence has multiple units, then a utility easement agreement, notarized and recorded with the clerk of courts, is required. Non-residential establishments require a floor plan showing the plumbing plan.
  4. ENGINEERS DESIGN: All New Onsite Sewage Treatment and Disposal System applications, in unincorporated Lee County, must be designed by a Florida licensed professional engineer and must include the required seal and signature.
  5. SURVEY:A copy of the survey must be included as part of the application package.

Once a complete application package has been received, the department will review to ensure all design features are included and accurate, then issue the permit. If there is additional information required before a permit can be issued, you will receive a letter detailing what that information is.
