The Florida Department of Health in Lee County (DOH-Lee) offers immunization services for children that include:
- Assessment of vaccine/immunization needs
- Assessment of immunization needs for school/childcare
- Free immunizations to children birth through age 18, including influenza
- Florida Certificate of Immunization (DH Form 680) for children with up-to-date immunizations.
- Immunization religious exemptions (DH Form 681) - individuals requesting a religious exemptions for children must apply in-person and bring the child’s birth certificate and parent/guardian ID.

No Cost Child Immunizations
DOH-Lee is a partner in the Vaccines for Children Program, offering immunizations at no cost for children up to age 18.
A child qualifies for the program if one of the following criteria is met:
- Enrolled in Medicaid, Medipass, or Medicaid HMOs
- No health insurance coverage
- Health insurance does not cover immunizations
- Child is American Indian or Alaskan Native
