Fort Myers, Fla. – The Florida Department of Health in Lee County (DOH-Lee) is announcing changes to their Vital Statistics and Environmental Health offices.

Vital Statistics 

Effective immediately, services will be provided by appointment only, during normal hours of operation, 9:00 a.m. to 4:00 p.m.

The Vital Statistics office is located at 83 Pondella Road in North Fort Myers. 

The DOH-Lee is partnered with VitalChek to provide an option for online and phone orders.  Orders can be placed online by visiting VitalChek.com and by calling 1 (800)-255-2414.

Environmental Health

Beginning Thursday, March 19^th through Sunday, April 19^th, Onsite Sewage Treatment and Disposal System (OSTDS) and Tattoo submittals will be relocated the Environmental Engineering building located at 60 Danley Drive, Unit #1 in Fort Myers.
