When a birth record has an error, it may be possible to correct it. You might need to show proof, like documents, depending on what you want to change and how old the person is.
If the correction is to the name of a minor child, both parents, if listed on the original birth record, are required to sign.

- Read and follow the Instructions for Amending a Certificate of Live Birth.
- Print and complete the Affidavit to Amend a Florida Birth Record.
- Print and complete the Application to Amend a Florida Birth Record – English/ Spanish / Creole.
- There's a $21.00 non-refundable fee for making corrections. This fee includes one certified copy of the corrected birth record.
Please submit your request by mail. The envelop must include:
- Completed Affidavit to Amend a Florida Birth Record
- Completed Application to Amend a Florida Birth Record
- Copy of your ID.
- Proper fee amount.
- Other documents, if required.
