Obtain a Copy in-Person or by Mail
You will need the following information:
The Florida Department of Health in Lee County is able to issue certified copies of death certificates for any death in the state of Florida from 2009 to the present. Death and fetal death certificates without the cause of death are public records. Anyone can get them without showing ID. Any death prior to 2009 must be requested from the Florida Department of Health’s Bureau of Vital Statistics in Jacksonville or through VitalChek®.
A legible copy of a valid driver’s license, passport, state-issued ID, or military identification card must accompany all requests. You must be a family member or legal representative when requesting cause of death. A completed Death Certificate Application (English or Spanish) should be submitted along with the appropriate fee.
If you are eligible, you can allow someone else to get the death certificate with the cause of death for you. To do this, you need to:
- Print and complete the Application for Florida Death Record.
- Print and complete the Affidavit to Release Cause of Death Information.
- Give the person a copy of your valid ID.
- All required documents, if applicable.
The original affidavit must be submitted with a completed Application for Florida Death Record.
