The Florida Department of Health in Lee County is able to issue certified copies of birth certificates for any birth occurring in Florida. Birth certificates are private records and can only be given to certain people if they provide a valid ID, any requested documents, and the correct fee. These people include:

  • Individual whose name is on the certificate (if they are 18 years of age or older)
  • Individual’s mother or father
  • A guardian or legal representative of the individual mentioned above, with proper proof or a court order
  • If the individual listed on the certificate has passed away, you’ll need to show their death certificate and be the mother, father, spouse, child, grandchild, sibling, or a legal representative of one of those people.
  • Once it’s been 125 years since the individual’s birth, the certificate becomes public record, and anyone can request a copy.
  • If you are eligible to get the certificate, you can give someone else permission to order it for you. Complete a Affidavit to Release Birth Certificate, provide a copy of your valid ID, and any other required documents. The original affidavit must be sent in with a completed application for Florida Birth Record: English | Spanish | Creole.

Commemorative Birth Certificates

Florida provides special birth and marriage certificates signed by the current Governor and State Registrar. They have fancy calligraphy, gold embossed seals, and are great for framing as family keepsakes. However, they can’t be used as legal proof of birth.
